Patient Safety Collaborative

The safety of patients sits at the heart of healthcare and is a critical thread running through all our work.  

ICHP, as the North West London Health Innovation Network, hosts the NW London Patient Safety Collaborative (PSC). Working in close collaboration with the NW London Integrated Care Board and system stakeholders, we are funded by NHS England to deliver national improvement priorities.

The PSC aims to address key safety issues and achieve better clinical outcomes and experience for patients across NW London by:  

  • Embedding quality improvement methods
  • Strengthening ways of working
  • Sharing good practice across the system  

Our team of experts have a range of experience and skills in quality improvement, systems thinking, stakeholder engagement and pathway co-design.

National priority areas

Our work is focused on four national priority areas. By March 2025 working with system partners we aim to:

Maternity and Neonatal:  

  • Improve the safety and outcomes for all women, babies and families across NW London and contribute to the national ambitions to reduce rates of preterm births from 6-8%, and the rates of maternal and neonatal deaths, stillbirths and brain injuries that occur soon or after birth by 50%. 

Medicines Safety:  

  • Reduce harm from opioids in non-cancer pain across NW London and contribute to the national ambition of 25,000 fewer patients prescribed opioids for more than three months, and 4,500 fewer patients prescribed high dose opioids.  
  • Inform the diagnostic phase of 2 national Medicine Safety pipeline projects [Psychotropics in Learning Disability & Falls inducing medicines in Frailty] 

System Safety  

  • Support seven NW London secondary care providers to embed and sustain Patient Safety Incident Response Framework (PSIRF) through the set-up of optimal conditions for patient safety and quality improvement. 
  • Support the testing phase of PSIRF pilot in primary care. 

Managing Deterioration:  

  • Reduce deterioration-associated harm by improving the prevention, identification, escalation and response (PIER) to physical deterioration through better system coordination across ICS and four NW London Trusts involved in the Martha’s Rule pilot.

How we work with system partners

  • Discovery: Collate and analyse complex to better understand challenges to inform meaningful improvements.  
  • System convenor: Establish communities of practice and facilitate shared learning events.
  • Horizon scanning: Identify and disseminate good practice (local and national)
  • Implementation: Quality improvement, culture & leadership, system thinking, pathway mapping & co-design, development of bespoke resources.
  • Evaluation and impact: qualitative and quantitative (includes PPIE) measurement plan and publication of case studies.
