North West London Patient Safety Collaborative

Supporting safer care across North West London through collaboration, improvement and shared learning. 

Patient safety is fundamental to high-quality care. The North West London Patient Safety Collaborative (PSC) works with partners across the system to reduce avoidable harm, improve outcomes and strengthen the experience of care for patients, families and staff. 

Hosted by Imperial College Health Partners (ICHP), the Health Innovation Network for North West London, the PSC brings together providers, commissioners and system partners to deliver national patient safety improvement priorities in our region. 

We support organisations across North West London to make care safer by combining practical improvement expertise with system-wide collaboration. 

  • Embedding quality improvement methods in everyday practice 
  • Connecting teams to share learning, insight and good practice 
  • Supporting local delivery of national patient safety priorities 

Our team offers expertise in quality improvement, systems thinking, stakeholder engagement, pathway co-design and organisational or team-based culture change – helping partners turn evidence, insight and ambition into meaningful change.

Our current priorities

Our work aligns with the national patient safety strategy and current NHS improvement priorities, with a focus on areas where collaboration and practical support can make the greatest difference to outcomes and equity of care. 

For 2026/27, our priority areas include: 

  1. System safety – using learning from the Patient Safety Incident Response Framework (PSIRF) to support improvement. 
  2. Medicines safety – reducing the risk of harm from psychotropic medicines for people with a learning disability who are at risk of behaviour that challenges. 
  3. Managing deterioration – improving the prevention, identification, escalation and response to physical deterioration through the implementation of Martha’s Rule. 
  4. Maternity and neonatal safety – focusing on safer care for women and babies across key improvement areas listed below;
  • Optimisation and stabilisation of preterm infants 
  • Early recognition and management of deterioration in women and babies 
  • Reducing avoidable brain injury during childbirth

How we support partners

  • Insight and discovery – we gather and analyse information to understand challenges, identify opportunities and shape practical improvement activity. 
  • Convening the system – we bring people together through communities of practice, learning events and cross-system collaboration. 

  • Sharing and scaling good practice – we identify, adapt and spread effective practice from across North West London and beyond. 

  • Implementation support – we help partners apply improvement methods, develop pathways, strengthen leadership and co-design tailored approaches. 

  • Evaluation and impact – we support qualitative and quantitative evaluation, including PPIE, to understand outcomes and demonstrate impact.
