Improving population health through successful partnerships

Imperial College Health Partners are excited to announce that we have won a bid, in partnership with Cerner, OptiMedis-COBIC UK and NEL Commissioning Support Unit  to provide a new performance and population health management dashboard for NHS England and Integrated Care Systems (ICS) across the country. The consortium will be bringing a wealth of experience from across England, along with international knowledge from sites in the US (Memorial Hermann, Advocate Physician Partners) and Germany (Gesundes Kinzigtal).

Integrated Care Systems merge treatment (traditionally the domain of the NHS) with responsibility for population health, prevention (traditionally public health) and social care functions. They present new opportunities and challenges and, as they develop, there is a need for new data, intelligence and tools to manage their health systems.

This new performance and population health management dashboard will build on the current reporting system, which is largely focused on separate performance metrics. It will also provide a way to monitor activity, population health, identify and effectively target resources to improve patient outcomes by joining together disparate elements of the health and care system. It will enable more preventive solutions to healthcare through its ability to track, predict, intervene and manage conditions cost-effectively.

The programme is working collaboratively across national organisations and four first-wave ICS sites (Nottinghamshire, Yorkshire and South Cumbria, West Berkshire and Bedford, Luton, Milton Keynes) to develop the first iteration of the dashboard, with a view to roll out to other first-wave sites as the programme progresses.

“We’re delighted that through our strong partnership with NEL CSU, Cerner and OptiMedis-COBIC UK we have been able to win this bid to provide a new population health management dashboard, helping local systems to better understand their local population needs.  At ICHP we can support integrated care systems on most aspects of their development including knowledge sharing, capability building and building integration. We’re very much looking forward to taking this project forward with our partners, supporting the delivery of better, more joined up patient care in those areas, and we will be sharing our learnings as we progress with the work.” Dr Axel Heitmueller, Managing Director, Imperial College Health Partners.

“We welcome this partnership to drive towards real benefits for patients through evolving and refining our understanding of population health analytics, helping NHS England and emerging Integrated Care Systems to target healthcare resource and keep people healthy.”  Graham Crawford, Director of Analytics, NEL CSU

“The journey towards integrated care and population health management will be underpinned by powerful and actionable data analysis from across the health and care system. The combined expertise of this partnership and other innovating health and care systems, coupled with the capabilities of our HealtheIntent platform, will provide the insight required to support organisations across the country to improve the quality and efficiency of care for their citizens. We’re confident that this programme will become a shining beacon of actionable data analysis at scale, and we’re proud to be a part of it.” Dr Justin Whatling, Global Vice President for Population Health, Cerner

“It’s often said that what’s measured is what matters. It’s a privilege to be trusted by NHS England to bring our UK and European experience of cost-effective integrated care to help design and deliver the dashboard that will be used across the country to help NHS England, local systems, and, we hope, the public to whom the NHS is ultimately accountable to understand how well the NHS and its partners are using tax-payers’ money to improve health, quality and experience of care and the use of resources.”  Dr Nicholas Hicks, Optimedis COBIC UK
