Understanding intermediate care in North West London

Intermediate care services provide support for a short time to help you recover and increase your independence.

The North West London Networked Data Lab, a collaboration between Imperial College Health Partners, Imperial College London, and the North West London Integrated Care Board, funded by the Health Foundation, has been working to understand who receives intermediate care in NW London and how this care impacts hospital readmissions. This initiative aims to use linked data to better understand and improve the health of our communities.

As well as data analysis, we’ve worked closely with an advisory group of patients and carers to understand their experiences and shape our understanding of the data. Our advisory group have shared more about the project, including what they learned and what this means for patients and carers, in our new video.

We’ve also partnered with Healthwatch Westminster and Healthwatch Kensington & Chelsea who undertook more qualitative analysis, working with patients and carers to explore more around the quality and continuity of the care they receive. You can read their report in full here.
